Immunization Records Question

Abi, to the best of my knowledge, a vaccination card is not actually a legal document, merely an informational one. I can't say why yours would have been whited out so extensively, but I do know that occasionally I mis-wrote while filling them out (in my former life as a pediatric nurse) and had to resort to white out. The important ones, with stamps and all, are on an international vaccination record, showing that you have had the appropriate shots for going to a foreign country.
If your last Td was in 1994 you might want to consider getting a booster; they are recommended every 10 years. Tetanus is a nasty thing to get, and fairly easily avoided with the shot.

Okay. I'm relieved. Hopefully informational is all they need. It says "proof of vaccines". It's for CNA hospital work.

I'm going to give this to a private eye and have them find out what I am!!!
In 1995? Then you still need another. Or did you mean 2005? If you don't want to get one every 10 years you should at least remember to ask your health care provider about a booster if/when you get any sort of dirty wound - like stepping on a nail, or getting dirt into a deep cut.

You DO know that DTP includes Tetanus (that is what the "T" stands for?

Ask your school, but when I had to get immuzination records, I had a HECK of a time. Mainly the doctors office where I got them all said they destroyed my records! :somad I called the state registry and all they had on file was a flu shot 2 years ago. I ended up calling my high school (as schools won't let you in without proper vaccines) and they were able to give me all the info.

you will need a Tet shot, when working in health care field (real or for school work) they require you to be up on your immuzinations. So, I would get that worked up so you are not put off by it later on. They also may make you get another TB shot, but that may be up to the school/health building.
