I'm no doctor, but I give hearing tests for a living, which ocassionally has me looking into peoples' ears. It could be severe wax build up...I know you use a q-tip, but that doesn't always help. Like the others said, I would go to an actual Audiologist and have them looked at. They could do tests on the fluid, and really get to looking into it. There could even be a small scratch inside the canal that just can't heal. The skin in your ear is extremely thin, and lays directly on the bone, thus making it suseptible to small scratches.
