:gig I wouldn't add that last bit to the actual cover letter...
I don't have it anymore, it was on an old computer, but I did something generic for medical billing once. I mailed it to local offices and got numerous calls back, two interviews and a job. Sorry I can't be more helpful.

You can probably find out the hiring manager by calling the general number for a facility and simply asking for their name/title. If you can find anything specific about the facility (that's positive, LOL), it always good to show you've done a little research on them - and you can tell them how you could specifically benefit them.

How do I do that, Jamie? Quote them their webpage somewhere in the letter? I can't do that thingie that Auntie says I can't do. Ummm generalize?

Typically companies like to brag on themselves on their sites. If you can weave that in, it shows you're more than just applying at random. I've phone interviewed good technical folks who had no clue of the job they were applying for. At least one guy was honest in saying he just needed a job because his student visa expired and he didn't want to go home. :D

Do you have a website or two you could PM me for an example?

You should be able to call the general number and simply ask to whom to send your employment inquiries.

In researching your organization, I was impressed by the amount of information you provide your current and potential patients. Keep in mind, you need to convince them how WONDERFUL you would be for them to hire! :)
